In addition to the Delta Healthcare Providers’ Credential Checklist, some facilities may require additional documentation. These may include state-specific background checks, fingerprinting, and updated immunization records such as MMR, Hepatitis B, Varicella, TDap (within the last 5 to 10 years), and Influenza. Other possible requirements include a 2-step TB test or TB QuantiFERON, competency testing or exams, fit testing, a physical exam or health statement completed within the past year, and copies of all current state licensure cards with license numbers and expiration dates. Additionally, facilities may request a copy of your current ASHA card (if applicable), any client-specific documentation, and a copy of your diploma (if available).
