What credentials are required for employment?
What is required can vary depending on whether the facility has additional requirements outside of our Credential Checklist. The items Delta Healthcare Providers requires to have on file are determined by Joint Commission and the government. These items include, but are not limited to:
- Copy of acceptable documents from list on I-9 (List A or List B or List C)
- Copy of CPR BLS-clear copy of the front and signed back of the card (Typically through AHA or ARC)
- TB skin test within the past year
- Hepatitis B Declination/3 vaccinations/1 titer
- Copy of voided check or official bank document
- Copy of Auto Insurance (if applicable)
- 3 References that have observed you in a clinical setting in the last year (1 must be a supervisor)
- 10 Panel Drug Screen (We will set this up for you after being placed)
