What credentials are required for employment?
The required documents may vary if a facility has additional needs beyond Delta Healthcare Providers’ Credential Checklist. However, certain items are always required based on Joint Commission and government rules. These include: a valid form of ID as required by the Federal Government’s Form I-9 to confirm legal authorization to work in the US; a clear copy of the front and signed back of a valid CPR BLS card; a TB skin test done within the past year; proof of Hepatitis B vaccination status, a voided check or official bank document, three professional references who have seen your clinical work in the past year (one must be a supervisor); and a 10-panel drug screen, which Delta will arrange after placement. You will also need to complete Delta Healthcare Providers’ Human Resources new hire paperwork.
